PARKING: A note about Diamond Parking in regard to the meters - You don't need to register in the parking meters to get the two hours free, they only register when they have been there for over two hours and need to pay.

- All NEW PATIENT visits have a $49 deposit required to reserve your time with one of our providers.*
- The $49 deposit is NOT the cost of your visit. Your initial 50-minute provider visit will be billed to your insurance or available at a cash rate of $250 (due at the time of service). The $49 deposit will be applied towards your first office visit.
*Deposits are fully refundable with at least 48 hours’ notice. Cancellations within 48 hours and missed appointments are non-refundable. By submitting payment, you acknowledge and agree to these terms.
Please note if you are a new patient, you will not be able to schedule a returning visit and will need to follow the steps for a new visit.
